• Photocopy of High School Report Card (for undergraduate) or Transcript of Records (for Medicine) with General Weighted Average (GWA) / General Point Average (GPA)
• National Medical Admission Test (NMAT) Score of 80% and above (for Medicine)
2. Academic Scholarship
• Regular student
• Filipino citizen and resident of the Philippines
• Minimum grade requirement per subject:
– A grade of 85 per subject for undergraduate applicants
– A grade of 79 per subject for College of Medicine applicants
• General Weighted Average (GWA) qualification
– Undergraduate Colleges – A grade of 90 or higher
– College of Medicine – A grade of 87 or higher

3. Financial Scholarship
• Regular student
• Filipino citizen and resident of the Philippines
• Annual family gross income of 200,000 pesos or less for undergraduate and 400,000 pesos or less for College of Medicine
• Minimum grade requirement per subject:
– 80 per subject for undergraduate
– 77 per subject for College of Medicine
• General Weighted Average (GWA) qualification:
– 85 or higher for undergraduate and Medicine
• National Medical Admission Test (NMAT) Score of 70% and above (for Medicine)
• Has passed the background check and interview of the Collegiate Scholarship Committee
• Letter of Intent
• Certificate of Moral Character
• Most recent Income Tax Return (ITR) of parents or recent personal ITR if applicant is a working student/ Affidavit of Non-Filing of Income Tax Return (ITR)

4. Service Scholarship
• Regular student should be at least on the 2nd year of playing as varsity or 2nd year of active membership and participation with the institutional chorale of DLSHSI
• Officers of the Institutional Student Council (ISC) (President, Vice President for Academics, Vice President for Activities, Vice President for Operations, Vice President for Finance, Vice President for Publicity and Mobilization, Executive Secretary, and College Assembly Presidents); Library Student Organization (LSO) (President, Vice President, Secretary, and Treasurer); Kollaborative Movement (KM) (President, Vice President, General Secretary, and Treasurer); and Saringhimig (SH) (President, Vice President for Internals, Vice President for Externals, Secretary, Treasurer, and College Representatives)
• Winners in international, national, regional, and provincial sports and choral competitions
• With good academic standing with grades not lower than 80 for undergraduate applicants and 75 for Medicine.
• General Weighted Average (GWA) of 83 or higher both for undergraduate and Medicine applicants.
• Has a rating of 80% based on the Varsity/Chorale Point Scoring Checklist of the Director for Sports, Culture and Special Events

5. Gawad Lasalyanong Edukasyon
• Regular student
• Filipino citizen and resident of the Philippines
• Graduate of Science schools and/or biggest public schools (whose Principals are ranked as Principal 3 or 4) in terms of the school population in the Province of Cavite, Metro Manila, CALABARZON, and other areas in the country.
• Top three (3) of the graduating batch
• Winners of the Cavite Search for Science Survivor-Wizards
• Top 5 examinees of the DLSHSI College Admissions Test (CAT) with a General Average of 90% or it’s equivalent rating in the other public schools during their fourth year
• Result of the College Admission Test (CAT) of 90 percentile rank and above
• Letter of Recommendation from high school principal certifying status as Valedictorian, Salutatorian and First Honorable Mentions

Requirements and policies for this type of scholarship are set by the sponsoring organizations and are evaluated by the offices concerned but the applicants are required to file their applications at the Office of the Director for Academic Services through the Scholarship Program and Development Center.
